About the Role

We are looking for a Population Health Support Specialist to support healthcare providers by ensuring patients, providers, and clinics are fully prepared before every appointment. This role focuses on chart preparation, patient outreach, care gap identification, follow-up coordination, and population health initiatives to improve patient outcomes and operational efficiency.

This is a non-clinical role that works closely with providers, care coordinators, nursing leadership, and scheduling teams to ensure patients receive timely, high-quality care.

Key Responsibilities

  • Review upcoming patient appointments to ensure charts are complete and visit-ready.
  • Identify preventive care opportunities and chronic condition care gaps.
  • Review labs, referrals, imaging, and follow-up requirements before patient visits.
  • Flag missing documentation, referrals, or clinical information for the appropriate teams.
  • Contact patients to schedule follow-up appointments, preventive screenings, laboratory work, imaging, and referrals.
  • Document outreach activities and patient interactions accurately in the Electronic Health Record (EHR).
  • Support care coordination by tracking outstanding patient needs and ensuring appropriate follow-up.
  • Assist with referral management and provider-requested follow-up activities.
  • Maintain accurate tracking tools related to care gaps, outreach, and quality initiatives.
  • Enter and maintain accurate documentation within NextGen and other clinical systems.
  • Support quality improvement initiatives and audit readiness.
  • Maintain HIPAA compliance and patient confidentiality at all times.
  • Collaborate with clinical and operational teams to improve patient access and continuity of care.

Requirements

  • Able to work in office: Islamabad– (Non-negotiable)
  • Able to work full-time: Monday-Friday, 8:00 AM to 5:00 PM PST – (Non-negotiable)
  • At least 1 year of experience in US healthcare environment, including one or more of the following
  • Experience working with Electronic Health Records (EHR).
  • Strong organizational, documentation, and time management skills.
  • Ability to work independently while meeting productivity goals.

Preferred Qualifications

  • Experience working with Electronic Health Records (EHR), preferably NextGen.
  • Experience supporting quality measures, preventive care, or care gap closure initiatives.
  • Previous experience in an FQHC or Patient-Centered Medical Home (PCMH) environment.
  • Bilingual English/Spanish is a plus.
