House Calls Home Care is looking for a motivated, organized, and detail-oriented Intake Coordinator / Care Coordinator to join our growing team.
Key Responsibilities
• Manage patient referrals, admissions, and intake processes
• Coordinate patient care services and staff scheduling
• Communicate with patients, families, caregivers, referral sources, and clinical staff
• Verify and maintain required admission documentation
• Ensure accurate data entry and record keeping
• Assist with caregiver staffing and case coordination
• Monitor ongoing cases and ensure timely follow-up
• Support daily office operations and customer service activities.
Requirements
- Minimum 2 years of experience in Home Care, Healthcare Coordination, Intake, Scheduling, or related fields
- Excellent English communication skills — both written and spoken
- Strong organizational, coordination, and multitasking abilities
- Proficiency in Microsoft Office and computer-based documentation
- Professional, dependable, and compassionate work ethic
- Must be willing and able to work night shifts.
